I can't login - says "login failed - AUTOSUSPENDED"

    Just finished creating a form and now it won't let me login... didn't log out... just says 
    "login failed - AUTOSUSPENDED"

    Upon review of your account it was found form:

    http://www.jotform.us/form/73305730174148 is requesting passport number information.


    Please take note that collecting sensitive information is not allowed here at JotForm. I recommend you check the Terms and Conditions.


    We need to delete the guilty fields so that we can reactivate your account. If you really need to collect the Passport information, then you need to have the following in your form:


    1. An agreement stating the usage of sensitive information. You can use the "Text" tool. You may also use the Short Scrollable Terms widget or Terms & Conditions widget.


    2. Digital signature field. You can use the E-Signature widget or the Smooth Signature widget.


    We reactivated your account and your form is available. Please make the appropriate modifications if you need to collect this information.

    Allow 30 minutes in order for the system to reset and recognize your login.
