Image Choice Question not appearing on PDF Form Creation

I have a few image choice questions on my form. When I go to Create PDF Form, the PDF does not include those questions. How can I get those questions to appear at all and especially with the images?

I have attached the URL to the form as well as the screenshot of the first page that would include the image choice questions, but it does not.

The Image Choice field is a widget and the widgets are not available in Fillable PDF forms.
