I've had a few different complaints recently about my pages crashing and/or not working properly.
I've also had someone tell me they thought they submitted a form, but it did not go through.
Can you help me troubleshoot and see what the problem(s) may be?
This is specifically for my new patient intake questionnaires (2 of them).

Apologies for the inconveniences this may have caused to you.
I can see your form is a bit long and the issue might be related to the continue forms later feature, may you please disable such feature?
If the problem persists, please let us know.
