Is this the right URL (http://www.gofindfree.com/advertising.html) of the page were the form is placed? And is this the changes your site visitors should see (http://www.myjotform.com/22602148912549/) ? If yes, upon checking your page source, I can see that you have this URL on your embeded codes:
http://submit.myjotform.com/submit/22602148912549/ , please replace the text "submit" to "www", so this means that from your form URL it becomes;
Jotform is currently having an issue with updating forms, so changing "submit" to "www" will help.
Hope this helps.

Apologize for the inconvenience that may have caused. Regarding on this issue, have you tried already embedding your form code into your webpage using iFrame method? If not, please give a try and do let us know if using iFrame method fixed the issue or not so we could further assist you.
