The name of the Form is different than the name on the tab at the top of the screen

    Hello, I noticed a problem on several of my forms. When I open a form, the name of the form is not indicated at the top of the tab.  The name of another form is listed instead.  I have included a picture of this and highlighted the name on the tab.  The actual name of the form is Anti-Harassment Training2_3. I have been having several problems with my forms lately.  

    Also having error problems on this form, message reads 

    There are errors on the form. Please fix them before continuing.

    Can you please see where the errors are coming from and fix if possible. 

     

     

    Please help.

    You can change your tab title from your form settings. Go to your form named 'Anti-Harassment Training2_3.' . You can change your title from 'Page Title' under Form Settings as seen below.
