Also trying to fill in the form I cannot move on from the first page.

I think the issue has to do with the custom link, the form loads an old version when you use the custom link, but loads updated when you load the default link: https://form.jotform.co/61076261683861
In fact, the form does not have a custom link, please try recreating it: https://www.jotform.com/help/261-How-to-Create-a-Custom-URL-of-a-Form
