How to set-up a supplier turn-in order form?

Hi Guys,

Just registered to test out JotForm and wanted to set-up a supplier turn-in ordersheet, for my members to use. I wanted to know if and how I can place elements horizontally in one section.

I want to build a layout that shows on one line/section a product image/description/pricing etc and a quantity box, as I may have a lot of products from one supplier.

Also if this can be done, can I then have a submit button that sends the order off to a nominated email address, or use something like Zapier to achieve this?

If I understood right, you would like your products to be shown on a horizontal line?

Not sure if that would look user-friendly since the width of the form should be increased in that case.

Regarding the sending the submitted data to a nominated email address, please follow: https://www.jotform.com/help/25-Setting-up-Email-Notifications, you can specify any email address as the recipient. You don't have to use Zapier for that.

Hi Guys,
I am trying to work out how to send an email notification, which you will see I have set-up for the test order form. I am happy with the format, however I do not know how to set-up the order form so the product description is populated with the order quantity. For some reason when I select the products from the form fields it only shows the order quantity and not the product details.
How do I set-up the turn-in order to show the product details and qty in the email template?

Since you used Text fields to show the description of the field, you have to enable that field to show in submissions and to do that, please go to Submissions:

And check "Show Headers and Texts"

That will show all description fields.

Hi Nik,
Did what you have said below however I am not getting the description as a form field I can use on my emails, so my email notification is still only allowing me to show the carton quantity field. See screen shot below.

About the Text field on your notification email, you need to add it manually. You can also customize your notification email content. Here's a quick screen animation sample that you can follow:

Hi John,
I have attached a video showing my issue. I include the Alcovit Form fields into my email template, and all the email shows when it goes through is the carton quantities. It does not bring across the product descriptions despite me ticking it in the set-up.

But I see the problem.

First of all, I don't see this form working properly with the setup you have.

And the issue is that you used Text field to name the products:

To be able to see Text field in the email, as my colleague said, you have to manually copy the text and paste it in the Notifier. That leads to the problem, you have to copy all product names to the Notifier, meaning they will all show no matter if they are selected in the form or not because that's how the Text fields work.

So I would rather place the text in the label of the drop-down field like this:

If you do it in that way, the product name and quantity will show in the email Notifier only if the product is actually selected.

This is starting to frustrate me.
I have now revised the form to just a simple form see below.
I have then set-up the notifications as below
I have then made the form live and placed an order for 4 cartons and got this message in return.
CAN YOU SEE THE PROBLEM!!! 4 CARTONS OF WHAT??
