How can I obtain a signed and dated W-9 form from JotForm?

My name is Tiffany Goines and I am the Office Administrator with Coastal Alabama Community College in the Adult Education Department.  We currently have the Bronze plan ($19/monthly) with JotForm which our Instructional Coordinator is paying for out of her personal funds.  

To correct this, we are looking to have JotForm added to Coastal Alabama Community College vendors list.  This requires our obtaining a  signed and dated W-9 Form from JotForm.  What do we need to do to obtain this required document?
