    Hi

I need to create appointment forms for school parent teacher meetings.

Is there a template for this that can avoid double bookings?

    Thanks

To avoid double bookings you can enable Unique Submissions or Unique Questions.

Unique Submissions,  filters cookies and Ip address of the user to check if he/she has submitted the form already and preventing them to submit again.

Unique Question, filters a field either email or Name so that it will detect if it has been used already and prevent the form from submitting.

You can go to Form Settings>Settings and scroll down until you find Unique Submissions and Unique Questions

At the moment, we have a long list of multi-choice times for each teacher in the school (12). This makes a very long form. Is there a way to do this more efficiently? Thanks

Also- is there a way to limit the number of choices that a parents makes? That is, to book e.g. 5 teachers only?

You can try to add a page break on your form so your form will be divided into pages instead of one long page.

You can also try to add a Checkbox field on your form with the teacher names listed on it so your user will select first the teachers then you can show/hide the appointment slot depending on the user's selected teachers. For more information on conditions, you can check this guide: Smart Forms Using Conditional Logic.

If you have a checkbox field that has the teacher list, then you can limit the number of teachers the user can select.
