The submissions below are not in the email activity log for our account, they were never received by us. It seems that every few months we have a problem with a number of these not being sent.

May you please provide us the form where you're experiencing the issue?
Also, as I can understand your issue, you're not receiving the submissions via email and nothing is recorded in the email history for your account, but you're seeing the submissions on your submissions page, is this correct?
Please provide us with your form/s we will be happy to have a look on them.

We are not receiving the submissions via email and nothing is recorded in the email history for our account, but we are seeing the submissions on our submissions page again. This time, it is submissions sent in on 10/25/17. The form is https://www.jotform.com/build/33533220062440

Please note that your notification is conditioned to be sent, meaning that if none of the conditions are met, it will never be sent. Upon reviewing your conditions, there are many that are incomplete:
Please make sure to complete your conditions, and that you will not leave the chance on your form filling process, that none of the conditions will be met.
