Why am i still locked?

    Hi,


    We have been locked out for about 2 weeks now.  I sent a message with the reason we were collecting the information we were.  We are an employer who is using jot form to collect new employee information.  That is why we are asking for SSN, etc.


    When can we get back in and continue?

    Upon review of your account it was found form:

    http://www.jotform.us/form/73243612249151 is collecting SSN and banking information without a mandatory signature field from your customers. We understand that you operate in this line of business but you need to follow the rules to make it possible to obtain this information. Banking information is also allowed with the appropriate signature field.
