Unable to view the existing contact us form

This website has been developed by previous developers, we got access to modify the existing website. We try to modify the existing contact us form in the karakosuits website. after we save it we are unable to view the form. We have the script written in the editor below but we are not sure what went wrong in the source. Can you please check in your record/database what exactly source was there previously? We have access for Magento platform from which website has developed.

Thanks for your reply... I tried using this script which you provided but unfortunately its not working...But i am sure the source number that is 71737193614965, there is something wrong with this number maybe missing or got replaced. Any idea how to figure out the previous source code prior to make changes?

This is actually the form id associated with the form. I have checked and found that, this form belongs to another user.

You can clone this form to your account and try embed it again in your website.
