How do I know the form is linked to my Paypal account information?

You should be able to identify in the Payment Settings if the form is integrated properly to the payment processor. I checked your form and I can verify that it is not yet connected to PayPal Pro. Here's a screenshot:

You may also click the "Verify" button after entering your credentials.
