The form layout seems to have changed

The form layout seems to have changed for me from a single long form that I could break into multiple pages if i wished into a series of questions that appear one at a time. Is there any way to go back to how they looked earlier today? We use the forms for a checklist, and don't always answer all of the questions sequentially. This new format makes it much more difficult for our users to fill out.

You can change it from the form settings.

Go to Settings->Form Settings->Show More Options

Scroll down to the option called "Form Layout" and select "All Questions in One Page" if you want the form in a single page.
