I received your e-mail response to my previous inquiry and have gone through all of the suggested steps to see if an e-mail was sent. Under the specific form (Carol Carter Workshop - May 2 & May 3), there are no submissions listed. Someone completed the form on December 4, and the respondent assures me that she received an e-mail confirmation after submitting the form.
When I checked my settings for an e-mail history, there are no entries at all although I have received 4 other notifications on other form submissions. I'm not sure why they don't appear in the history.
I would appreciate having someone look into this. Thanks!

Since you didn't provide any form ID and I didn't find any form named "Carol Carter Workshop - May 2 & May 3", I tested the form named "Carol Carter Registration - May 2 & 3" (Form ID is:73317784162157).
I did test submissions to your form and also I cloned and tested but I didn't replicate the issue.
According to your mail history, there is no submission other than my test submission.

If so, could you please provide us form ID?
