We need a form that allow for uploading file and payment via PayPal

I have a need to create a form that will...

1)  Allow people to upload files (specifically pictures)

and

2)  Make a payment via PayPal

Is there an option or template in JotForm that would include both of those options?

Allowing users to upload files is as easy as adding a file upload field to the form:
