Why is there an application for Student Incubator in my list of forms I created?

How did Student Incubator Application get into my list of created forms?

Please remove that application form and explain how and why this happened?

It makes me feel that your site is not secure if someone else can insert their form into my list of created forms.

The form you are referring to is one of the templates in the Form Templates Gallery. Here's the link: https://www.jotform.com/form-templates/student-incubator-application.

It is possible that you accidentally or unintentionally click the "Use Template" button. If you do this, a cloned form will be created in your account.

You can delete this form on My Forms page. Just select the form and then click the "Delete" button.
