Integration with MailChimp was successful and Jotform submission successfully populated email and name fields in MailChimp list. However, address field was not populated.
Integration menu recognized "Address" from MailChimp and "Home address" from Jotform form, but field was not populated. Neither is a required field, but was filled out in submission.

I can see that you are using the Address field in the form elements. Were you using the same address field type on your MailChimp form? This is the only field that matched with the address field in JotForm.
Please try to re-integrate your form and let us know how it goes.

“Address” is the name of the field on both the JotForm side and MailChimp side but addresses still not getting through to MailChimp list. Removed integration and re-intergrated but same result.

I have tried to reproduce the problem but I couldn't. It worked just fine for me.
So I created a new form, added 3 fields, full name, email and the address field. I integrated it with MailChimp with the same fields name, email, and address. After a submission, the address field is recorded as expected.
Here's my test:
I was able to reproduce it now. I think you're referring to the existing submissions sync into your MailChimp list. I discovered it after re-integrating my form with MailChimp.
So here's what happened, incoming submissions record the address value as expected.
However, existing submissions synced into the list doesn't send the address data. The same thing happened when you re-integrate or edit the integration. It clears the existing address data.
I will now forward this to our developers so they can fix this as soon as possible. We will let you know here once this is resolved.

The bug that occurs when editing the integration should be fixed now.
