I have checked your form and it is working fine at my end. I was using Internet Explorer 11 to replicate the issue but it is online and working.
Kindly check your anti-virus if jotform domain is blocked or reset your browsers cache.
If the issue still persists, can you tell us what version of IE you are using and what browsers are not loading?
