Uploading an excel form i have created into a form

is it possible to upload an excel form i have created? it is a sales assessment form. i want to know if i am able to do this does it populate the areas that need to be filled out? im sure i will have more questions but i am just trying to get more details on it now. If this does end up working i will upgrading our account to create more forms. please let me know.
