I assume that this is the correct form you are referring to http://www.jotform.us/form/73405231984154 ?
I have checked it just now and it seems there is no problem with the Paypal Integration being listed twice. Please see screenshot/GIF image below.
If the form is correct and it doesn't look like the screenshot I attached, please try clearing your browser's cache by following the guides on this link http://www.refreshyourcache.com/en/home/
Let us know if that is not the form which is having the issue and kindly give us the correct URL/ID.
