Right now this form works as I would like, but user feedback indicates the 1-4 radio choices are confusing.
I would appreciate your input on how to use jot form to be more explicit to user in creating the following work flow
After page 1 of form is completed and submit is clicked
Click your status in the sequence of feedback *
1. I am resident and informing attending of my goals
2. I am resident and did case - now will self reflect
3. I am attending directing resident reflections
4. I am resident reviewing my attending comments
Once user returns to form , page 2 is presented and #2 could be clicked
Then
Once user returns to form , page 2 is presented again with hidden fields revealed and #3 could be clicked
Once user returns to form , page 2 is presented again with hidden fields revealed and #4 could be clicked

I apologize, but I am not sure if I have understood your workflow correctly. It seems like you are looking to display fields based on previous answers only if the submission is in edit mode, is that correct?
