Right now this form works as I would like, but user feedback indicates the 1-4 radio choices are confusing.

I would appreciate your input on how to use jot form to be more explicit to user in creating the following work flow

After page 1 of form is completed and submit is clicked

Click your status in the sequence of feedback *

     1. I am resident and informing attending of my goals

     2. I am resident and did case - now will self reflect

     3. I am attending directing resident reflections

     4. I am resident reviewing my attending comments

Once user returns to form , page 2 is presented and #2 could be clicked

    Then

Once user returns to form , page 2 is presented again with hidden fields revealed and #3 could be clicked

Once user returns to form , page 2 is presented again with hidden fields revealed and #4 could be clicked

I apologize, but I am not sure if I have understood your workflow correctly. It seems like you are looking to display fields based on previous answers only if the submission is in edit mode, is that correct? If correct, please see this guide: https://www.jotform.com/help/389-How-to-Show-or-Hide-Fields-on-Edit-URL
