Form (result) not shown as designed

The form seems like not really shown what have I designed

1)Logo not shown,

2)some item, I have hide it, it still shown.

3)Logic sequences doesn't work too.

I have checked your form and it is working fine at my end.

Logo,conditional logic and show fields condition are working.

Kindly take a screenshot on the errors, so we can address the issue.
