My account has been suspended

    I need help with my account because said that my account has been suspended and I don't know why...

    Can you help me with that.

    Upon review of your account it was found form:

    http://www.jotform.us/form/73136882939167 to be collecting SSN# , which is not allowed unless you add a signature field of customers' acknowledgement to give out this sensitive information.
