Apparently, that form is under a guest account, meaning you were not logged in when you created that form.
Guest accounts rely on browser cookies, and the moment something happened to these cookies (cleared, overriden, etc), you will lose access to this form and the submissions made to it.
The only way to restore access to the form is to register an account using the same email address entered in the form's email alerts which is email@example.com. After registration, forms from your guest account will be automatically carried over to your registered account. If you can't view this form, please let us know so we can help you further.
