I'm not sure if I understood your question correctly.
Do you want to see the headers of your pdf submissions?
You must go to your form submissions
then click the gear icon on the right side after click Show headers and text you will now see your header image.
Please let us know if this is what you were looking to achieve and especially if not.

Thanks for the quick response, in part I managed to solve with your help, but I would like to make the images of the form appear in the pdf.
In conclusion, the pdf must be equal to the form including images and with the same formatting is it possible ???

The option to include headers and text will include the images from the form in their appropriate places. It does not make the PDF look exactly like the form. The PDF output can be customized further:
