How can I bring 2 accounts down to on1?

I am with the American Board of Radiology and we currently have two Jot Form accounts, one under the name of Susan Marsala and one under the name of Tina Henson. We are currently being charged for two accounts and we have active forms on both accounts. We would like to go to just one account, the one under Tina Henson and we would like all of the forms from the Susan Marsala account moved to the Tina Henson account. Can you please help with this?

What is the account username where the forms will be transferred from? Please note that the request needs to come from the account username where the forms are currently hosted. Please do the following:

1) Log in to the account username where the forms will be transferred from.

2) Open a new thread in our support forum.

3) Specify the forms that will be transferred, or let us know if it's going to be all of the forms.

4) Let us know the account username that will receive these forms.
