I can't figure out why, after being filled out, my form will not submit. When the Submit button is clicked, it does not go to Jotform or to the specified recipient. Can someone please look it over and let me know what needs to be corrected?

The form itself looks to be functional. Try replacing the source code embed with your forms iFrame code:

Doing so will also fix a few of the formatting issues present in the embedded form.
