Why did my forms background and heading text colour change??

Hi there, 

My heading text has changed colour to white? There has also been a pattern added to the background (which I have since removed) 

There has been some other changes to the form too.. How has this happened, has anyone else had these issues recently?

I have checked your forms and your headings are colored black with white background and your sub headers has a yellow background.

May I know what was the background color of your header that you had and  other changes that occur so I can assists you?
