How can I setup my form with Square

Hi! I am wondering how this works with Square? I would like an app that pops up before they pay, if they are renting a golf cart to fill out. I would like it to be a rental agreement for the use of the golf cart. Is this possible? And does it charge for every person that uses this form, just one form used in our application? Thank you!

FREE - 1 payment submission per month

BRONZE - 10 payment submissions per month

SILVER - 100 payment submissions per month

GOLD - Unlimited payment submissions
