When I preview the form, I only see part of the form

I've created a form for the first time. When I am editing it looks the way I want it to. When I preview I only see part of the form and when I publish and use the link I see even less of the form.

I checked your form and I was able to see all the fields including the Submit button. Here's a screen capture: 

Is this form embedded on a webpage? If yes, then please provide the URL of the webpage where the form is embedded so that we can further investigate the issue.
