I have investigated situation but there is nothing wrong with your website and SSL certifications.
Could you please try to see directly "https://secure.jotform.us/form/22240179447150?websiteUrl128=MedicalAlertDiscountWeb" to see if you get any certification error ?

Thank you for the people that replied so very quickly. Ya'll are amazing. I also contacted my security certificate company and they gave me this feedback: =================== [they placed an order.....] I personally did not receive an error, but perhaps I was not using the same version of IE as
you. However, using Mozilla FireFox and a plug-in called "FireBug", I was able to find that
the order form's "Success" page loads an image or two via a https://submit.jotform.us/ URL.
According to the following geocerts.com "SSL Checker", that secure URL (hostname of
submit.jotform.us) does NOT match the SSL certificate that is installed for it. The SSL
certificate presented only seems to support secure.jotform.com and jotform.com. Here's a link
to that online tool:
If you perform the same test on your domain, www.medicalalertdiscount.com, you will see that we
have installed it correctly for you. You may need to bring this error to the attention of the
jotform.us support staff to see if they can fix it for you. If their images were referenced
via secure https://secure.jotform.com/ URLs instead, the issue would be resolved.

The thank you page will only display after a successful submission, and so, it should not affect the form's ability to accept submissions.
If this is bothering you, try setting your form to use a customizable Thank You message as oppposed to the default Thank You page.
