Can you please help us fix our account? We seem to have exceeded our monthly form views limit however this is not possible as the forms do not get used that much and the pages they are on get very minimal page impressions.
I don't think we need to upgrade our plan, can you please advise?
Currently the only form we have live is this:

Thanks so much, I've now enabled the footer and also cleared the cache, republished etc, however our form is still unavailable.
Look forward to hearing an update from the back end team.

Yes, the form views exceeded again tremendously. I reset again and re-activated your account. We will inform you further through this thread.

When I check your form activities IP : 188.8.131.52 has the most activity and it is like spamming.
It worths to ask your web admin about this spam. I don't want to block that IP since that looks like your IP address.

OK my IT team have confirmed that is our external IP address, but we're not sure why it's constantly hitting this page. I've double checked my web statistics and the entire Colorado website only receives approx 25 - 40k page views per month. The page housing the form has received only 49 views since it went live.
Is there any chance we can exclude that IP address from our quota without blocking it?

We can't exclude form views. We can only ban an IP address to prevent spamming. Since this is your IP address you should contact your IT or web admin to identify this spam. It might be related with an internal daemon accessing this web site.
