How to dynamically add a group of people?

The jotform would first ask a simple question: How many people are in your group? And for an example, if they enter 8 the form would auto generate 8 fields for information needed like: full name, cell phone number, service/s needed, gender preference....etc?  if they say 6, it would auto generate 6 fields for info needed.  does this make sense? we own operate a large volume day spa and want to create a form that we would send to our clients if they are trying to book a large group.  Currently, we use pen and paper and its very unorganized.  We love jotform for many aspects of our business.

    thank you for you help

You can use the Configurable List widget, which will add dynamically the group of fields in separate rows, so the user can add as many as he wants to: https://www.jotform.com/help/252-How-to-Add-a-Widget-to-Your-Form

Please check this guide to learn how to set it up: https://www.jotform.com/help/282-How-to-Set-Up-the-Configurable-List-Widget
