How to use JotForm with Squarespace Lightbox

If you copy the lightbox straight from JotForm and place it in a Code Block in Squarespace it doesn't style the button properly. 

My main and only real reason for using JotForm is the Referrer Widget as it will help us identify that an enquiry has come from Adwords.

Has anyone managed to implement Jotforms properly via a lightbox?
