How can I change my form layout into the Old one.

I have a sign up form for dealers at my antique event ... however, instead of a signup sheet, it appears as a menu slideshow. Some of my vendors are older, and not terribly tech savvy. I'm afraid that most will bail on the signup, unless I can make it a single screen they can scroll down. How can I adjust the settings?

On the link you have share http://www.boldventurepress.com/paconsign.html It seems that you were able to figure out already on how to change the Form Layout using the Single Page style.

Anyway, You can do it in your FORM SETTINGS > click SHOW MORE OPTIONS > Scroll down to the last page and select the "All Questions in One Page" in the field Form Layout.
