How to add form and make it work within a closed Facebook group?

i created a forn for my facebook (group) for members to fill out then post in a post to the group. i need a stand alone fillable form to be used in the group by people. and not have to have it sent to anyplace. i made a form and scanned it to my computer. i need to know how to make it interactively fillable in the group.
