How to open form with mobile app?

I work at an amusement park and I've been trying to be able to have checklist and surveys for my supervisors to fill out during their workday. There isn't a Google Forms app so the only way to use that is to send out a link for them to click and take to a web browser. Most other apps are paid to use or have to use a pricing plan. 

My main question here is there any way to have my supervisors to have an app on their phones to just open, click a survey I created through JotForm and when they submit it, the responses come to me.

If I understand your question correctly, you want to access your form using a mobile app. Is that correct?

I would suggest you to please take a look at our "Add to Home Screen" widget and see if that fits into your requirement. This widget will add a shortcut in iPhone, iPad and iPod. When users click on the shortcut, it will open your form on their home screens.
