How can I create an driving fare estimate?

Hi, I've attached a picture to show you what I am going for? Can this be done in JotForm? I'd purchase a plan with you guys if I can make something very similar to what Uber does when someone wants to book with their service? If possible can someone help me with guidance on how to go about it?
