Is there an additional charge for HIPAA compliant/encrypted forms?

    Hello,

    We are working on a proposal for a client who will need several forms that are HIPAA compliant. We have read the knowledge base information about how to do this.

    However, is there an additional fee for the encrypted form & keys? Or will there be an increase in our monthly rate?

    No, there is no additional charge for using encrypted forms: https://www.jotform.com/help/344-Encrypted-Forms-and-How-to-Use-Them

    May we obtain social security numbers through the encrypted form? Our client who would like to have the SSNs on their loan application is in the financial lending industry, providing loans for business equipment and working capital.

    Yes, it possible to collect social security numbers if you use encrypted forms: https://www.jotform.com/help/8-Terms-of-Use 

    "Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information."
