Unfortunately, I was not able to replicate the issue you are referring to. The Subtotal and Total fields are showing properly in the form. It is calculating correctly as well. Here's a screen capture:
If you are still having issues, please provide more details so that we can try to replicate it.

I also see the Total$ showing up when I had a look at your form.
Can you please be more specific on which section of your form is not showing up? If you have a way to reproduce this problem, let us know how so we can have a closer look.

Can you please change the default value of Quantity to 0 or leave it empty. That is the problem why it is not calculating the cost per each print.

It was working fine with "1" as the default for quite some time now. This just started...

It seems you have now changed the default value as "1". I did check your form and I am unable to replicate the issue you are having. The calculation value seems to display correctly. Please check the screenshot below:
Have you embedded your form in any webpage where you are having this issue with? Please share the webpage URL and we will take a look. I would also suggest you to please change the default value to "1" and we will test this issue again.
