I have created a survey in Excel. How can I get something similar in JotForm?

    I am attaching a partial screenshot of survey questionnaire that I created.  The Screenshot shows a couple of headers:

    Leadership & Culture

    Customer Focus


    There are six such areas.  Each has 2-7 questions.  There are 3 responses to choose from, and the responder is expected to put a number in the column 'Response Score'.  He/she can also write comments in the 'Comments' column.


    Ideally, I would like to be able to see all individual scores and an aggregate by the six categories + an overall aggregate score.


    I would also like the respondent to also be able to see the scores.


    Of course, I would like to get the respondent's contact info.


    I would like to try the free version of the form initially to see how the responses look.  Once it is working, I will most likely go with the next level of the paid program.


    Can this be done in JotForm?  Can you guide me?  I am able to save the Excel sheet in html, but it does not get saved as a form, though.

    Unfortunately, what you are trying to create is not possible with Jotform in the sense that jotform doesn't have calculation function by fields. Sorry about that.
