Strange Cloned Form In My Account

HI,

I was just looking at My Forms page and I see a new form there or at least a clone of one:

 Clone of Form senza titolo 1.

I have no idea where it came from. Can you offer any suggestion?

I checked your account history logs (Logins and Form Creations), I see only one IP Address log in to your account and it is your IP Address. The form was created on December 21, 2017, at 5:07 am. 

Did you try to create or clone a form during that time?

Well that is very strange, my Spanish, and I presume this is Spanish, is as good as my Persian, so I have no idea what it means. I was making a clone of a form from a website about that time and was successful. How this came to be is a mystery of the 10 magnitude. 

I will also look at the history... and I will delete the form, which has no fields.

Hi,

After reviewing your history image I recall the first attempt at cloning created a form called Heading, which was not what I wanted, it looks like it created the clone at the same time and I think I know why. With that said this is no longer a mystery.
