My form is freezing on one of the questions

My form keeps freezing at the email portion. When you bypass it using the timeline scroll at the bottom you can get to the last page with all the fields filled out and submit it but it sticking on Please Wait...

I need this to work asap. I have a field team who uses this staff daily. Please advise.

I checked your form and I was unable to replicate the issue. I was able to bypass the email field without any issues. I have tested it with Google Chrome and Mozilla Firefox. Here's a screen capture:
