Is there a way to assign values to the multiple choice field?

    Is there an element that includes a Multiple Choice list AND a Short Answer (on the right side of each option) where parents can simply go down the list of teachers and input a donation?

    One good way of achieving this is to assign CALCULATION VALUES to the MULTIPLE CHOICE FIELDS. However, this would only work if there's a fixed amount of donation per teacher.

    If you need the donation amounts to be variable and would depend on the desired amount to be donated by the user, then setup your payment field to SELL SUBSCRIPTIONS, then set each and every teacher as a subscription with a corresponding input for a custom donation amount.

    The guide above uses a recurring donation but you can setup each subscription as a one-time payment by following this guide thereafter.

    To sum it up, this is how your workflow would be (all of which should be discussed in great detail on the guides I linked above):

    1. Setup your payment field to SELL SUBSCRIPTIONS

    2. Add each teacher as a subscription

    3. Set them up so they are all accepting VARIABLE PRICES

    4. Then, set the NUMBER OF PAYMENTS to 1 so it's just a one-time payment instead of recurring

    I encourage you to review the guides and look at the demo forms so you at least have an idea on how the custom donation would look like when used in conjunction with one-time payment subscriptions.

    This is brilliant!

    The only problem is that each parent can donate however much they want, and this will be a one-time payment. The workaround is only allowing me to do a one-time payment for a set donation.  Or, using the variable price only allows "Monthly" "Weekly" etc., options.

    Also, there doesn't seem to be a way that someone can select multiple teachers using the subscription method

    I don't think there's a way to achieve your needs using our payment fields. When SELLING SUBSCRIPTIONS, users can't subscribe to more than 1 subscription. Allowing multiple selections can only be done when SELLING PRODUCTS since users can typically buy more of something (as opposed to subscribing to something).

    And seeing that you're using the PURCHASE ORDER field, I'm assuming you won't be charging your users straight from the form. If you plan to charge them outside JotForm, I don't think my suggestions will work for your case. The PURCHASE ORDER field won't even allow you to set the NUMBER OF PAYMENTS since it won't be collecting payments to begin with.

    I'll tinker around a bit and see if I can come up with something. I'll update you on this thread regardless if I manage to find a workaround or not.

    If you have something else in mind, don't hesitate to let us know if there are other specific requirements you need.

    I checked your form and it is almost configured properly. You just need to change the Total Donation amount field to Form Calculation widget so you can pass it to the Payment Field.
