My probation review form does not work as it should. The report generated for HR Notification does not show the data filled by the employee in section 1. It only shows the data filled by the line manager in section 2.

I did check your form and found that you have added multiple notification email alert in your form. It seems your "HR Notification" email alert does not have all the form fields added. I would suggest you to please delete your email alert and add a new one. Please note that adding a new email alert will always include all your questions of form.
