Kicked out of account

Hello! I was just in the middle of creating a form when I was kicked out of it. I tried to log back in, but it's saying my account is suspended. This happened with the first form I tried to create. Can someone please (1) let me back in and (2) figure out how to ensure this does not happen again. I need to get an agreement to a client and don't have time for these interruptions.

    Thanks!!

I see that your account was automatically suspended by our system. This happens when our filter finds certain words in your form which may trigger such suspension. However, upon reviewing your form, I see that this is a false positive, and as such I have reactivated your account and whitelisted your form. You should be able to access your account again.

If you have other questions, please let us know.

    Thanks.

Thanks! This form contains standard terms that I will use in at least one other form - is my account whitelisted or just this particular form? I don't want to run into an issue again when I create another form. Thanks!!

We do not allow for the collecting of usernames or passwords through forms for any reason.  Doing so will result in the account being suspended.  Your form contains the following fields:

    http://www.jotform.us/form/73554375338161

    Email Platform Username

    Email Platform Password

It has not been whitelisted and will likely result in the account being suspended once again if those fields are not removed.
