Upon review of your account it was found that form:
http://www.jotform.us/form/73555179684168 is collecting social security numbers.
Such collection is prohibited under ourTerms of use
But it is allowed only if you add a signature widget in order for your customers to acknolwedge the collection of such information.
Please refer to the link below:
We have cleansed your form and reactivated your account. Please add first the widget then proceed to request the information in order for the system to recognize it is a valid collection.
Allow 30 minutes for the system to reset and be able to recognize your login.

manuelburkhardtReplied on December 22, 2017 at 2:43 PMOk that would have been nice to know as I was entering that field. Completely locking me out of the system without any warning was a pretty frustrating experience.
I’m going to cancel my subscription.
You need to find a better way of making that “violation of terms” more prominent, you need to give clients an opportunity to change it before locking them out of their own account.
