Every time we try to open a payment agreement or consent form the website does not allow all consent forms to show, when we complete the form the submit button is not available.
How can I resolve this issue?

The form is not that large so it should not be slow loading. When I checked the form on mobile, it did load quickly and the submit button was available:
Is the form embedded to a web page by any chance? As far as I can tell, there are not any issue on mobile with the form itself.
