Can someone take off the suspension on my account?

hello can someone take off the suspension on my account. I just reset my passwords.

Your account (dflcs123) is currently suspended. This form (Patient Intake Form) triggered the suspension. The patient intake process form has a field that is asking for Social Security Number (SSN). This is considered as a sensitive information. Collecting sensitive information is not allowed here in JotForm. Please check our terms of use.

If you really need to collect this information from your users, then you need to have the following in your form:

1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities

2. Add a signature widget where your users sign and agree to provide such sensitive information.

I have now activated your account since I can see that the guilty form is now in the Trash folder. Please "Purge" the deleted form in your Trash folder to avoid suspension in the future.
